Useful Forms
Parking Permit
Each year we are grateful to the Church of the Immaculate Conception for allowing a small number of our parents to park in the church car park. Allocation for these spaces is based on a number of strict criteria. You are able to apply in the Summer Term for the next academic year. Application forms are available from the School office and should be returned directly to the Parish office by the first week of August. Please find a copy of the form below.
Request for a Leave of Absence During Term Time
There are very few occasions where Headteachers in England are allowed to authorise children to be absent during term time. If you want to take your child out of school for any reason, including medical appointments, you need to complete an absence request form (see below). Please note that if the absence request is not authorised and the holiday is taken the Headteacher may request that the Local Authority issue a Fixed Penalty Notice. This is currently £60 per child per week per parent. This increases to £120 if paid after 21 days, and may result in legal action if 21 and 28 days. I understand that if I do not pay this it may result in legal action.
Scooter/Bicycle Licence Application
Children are able to bring a scooter or bicycle in to school as long as they first apply for a licence. The licence lasts until their last day of primary school, but may be taken away if the safety rules (explained on the form signed by the pupils and parent) are not followed. Please complete the application form and hand it in to the school office. Permits should then be tied to the relevant scooter or bicycle (and can be transferred between both).
Administration of Medicine During School Hours
On occasion, it may be necessary for your child to take medicine, for example antibiotics or Calpol, during the time they are at school. Staff cannot administer any medicine unless we have written permission from a parent. Please complete the appropriate form and hand it in with the appropriate medicine to the school office. Unfortunately, this does not include medicine classed as 'controlled drugs' which cannot be kept on school premises or administered by staff.
